From University Gatekeepers to National Standard

Until now, the pathway from MBBS student to licensed doctor in India passed through the examining authority of individual universities. A student at Osmania University sat examinations set and evaluated by Osmania. A student at AIIMS Delhi faced different papers, evaluated by different examiners, applying different standards. Both emerged with the same credential — an MBBS degree — that granted them the right to practice medicine.

The National Exit Test (NExT), introduced under the National Medical Commission (NMC) Act 2019 and being implemented across 2025-2026, changes that model. NExT creates a single national evaluation standard for all MBBS graduates in India — regardless of which university granted their degree, and regardless of whether they trained at an Indian or foreign medical institution.

The implications for professional examination evaluation extend beyond licensing. NExT is among the most significant standardisation exercises in Indian higher education history, and its design choices reflect a broader shift in how high-stakes professional competence is assessed.

What NExT Is and What It Replaces

NExT has two distinct steps:

Step 1 is a theory examination taken after completing the 4.5-year MBBS program, before internship. It covers clinical subjects across medicine, surgery, obstetrics and gynaecology, paediatrics, ophthalmology, ENT, and allied disciplines. Step 1 is conducted as a Computer-Based Test (CBT) administered on NMC-controlled infrastructure — standardised clinical reasoning questions delivered digitally across designated test centres nationally.

Step 2 is a clinical skills assessment, conducted after completion of the mandatory 12-month rotating internship. It tests practical competencies through Objective Structured Clinical Examinations (OSCE): standardised patient encounters, procedural skill assessments, and clinical reasoning cases evaluated against explicit, pre-defined criteria by trained examiners following structured checklists.

NExT replaces multiple layers of the current evaluation structure:

  • University MBBS final-year examinations — which were the primary qualification gate and varied considerably in standard between institutions
  • The Foreign Medical Graduate Examination (FMGE) — the screening test previously required for the roughly 25,000 to 30,000 Indian students who complete MBBS abroad each year, a group with historically high failure rates on first attempt
  • State medical council licensing processes — which added additional steps in several states before a graduate could practice
  • A single national examination, structured in two stages, now serves all these functions for approximately 1.2 to 1.5 lakh MBBS graduates annually.

    Why Computer-Based Testing for Theory

    The NMC's choice of CBT format for NExT Step 1 is not arbitrary. It reflects the same pressures that have driven digital evaluation adoption in school and university examinations across India: the need for assessment that is accurate, scalable, and auditable at national scale.

    Managing a theory examination for 1.5 lakh candidates through university-controlled written papers, evaluated by individual examiners, would introduce the full spectrum of problems that affect large-scale manual evaluation: examiner variation, totalling errors, mark transcription mistakes between the physical answer book and the results system, and limited post-hoc auditability when candidates challenge outcomes.

    CBT eliminates the theory evaluation problem in its most common forms. Questions with defined correct answers — including complex multiple-best-answer clinical questions — are auto-scored. Every student's response is logged at the question level, timestamped, and stored. Score generation is instantaneous and auditable. Statistical item analysis identifies questions that performed unexpectedly, flagging them for review before final marks are confirmed.

    The format also enables performance analytics that written exams cannot produce at scale. When a large cohort of medical graduates consistently underperforms on a specific clinical reasoning pattern — diagnostic reasoning for infectious disease presentations, for instance — the CBT database captures it at the item level. That data flows back to NMC's curriculum review cycle and to the competency framework that individual medical colleges are required to teach against.

    Equating Across Test Forms

    One technical advantage of CBT for a national examination is the ability to equate scores across different test forms administered on different dates or in different shifts. NExT, like JEE Main, is expected to be conducted across multiple sittings as the candidate volume scales.

    Equating ensures that a candidate who sat an examination on a given date is not advantaged or disadvantaged relative to a candidate who sat a different version of the paper. Statistical equating algorithms, routinely used in large-scale CBT systems, adjust raw scores based on the relative difficulty of items across forms. This is structurally impossible in a handwritten examination that uses the same paper for all candidates in a single cycle.

    OSCE and the Standardisation of Clinical Assessment

    NExT Step 2's OSCE format applies the same standardisation logic to practical competencies. In a traditional MBBS clinical viva — the format being displaced — evaluation depends substantially on the examining doctor's standards, questioning style, and implicit expectations. Two equally competent students at the same institution can receive different clinical viva marks based on which examiner they draw.

    An OSCE station specifies, in advance, exactly what the student must demonstrate. The examiner follows a structured checklist. The student's performance on each element is independently recorded. Inter-examiner variation is reduced by design, because the assessment instrument tells the examiner what to look for rather than leaving it to professional judgement alone.

    This is a significant reform for a clinical discipline. Medical competence is not reducible to CBT questions, but OSCE assessments have thirty years of peer-reviewed evidence behind them as a more reliable and valid measurement of practical clinical skill than traditional bedside viva examinations. The UK's Medical Licensing Assessment (MLA), Australia's AMC clinical examination, and Canada's MCCQE Part II all use OSCE-based formats for this reason.

    For the approximately 700+ medical colleges across India — including private institutions where clinical assessment standards have historically varied considerably — the shift to a national OSCE framework has direct implications for how clinical rotations, internship programs, and skill development are structured and documented.

    What NExT Means for Medical Colleges' Internal Evaluation

    NExT creates an external benchmark against which every medical college's graduates will be measured publicly. A college where students consistently clear NExT Step 1 at high rates is demonstrating the effectiveness of its clinical education. A college where students struggle signals a gap between internal assessment (which passed them into final year) and national evaluation standards.

    This dynamic parallels what CUET and JEE have created for undergraduate admissions: a national assessment that provides a reference standard against which university-internal evaluation can be calibrated. Medical colleges will increasingly align their continuous assessment, internal examinations, and OSCE practices with NExT's framework — not because regulation mandates it, but because NExT pass rates are becoming an institutional reputational signal.

    The data trail matters here. Medical colleges that maintain digital records of:

  • Student performance across continuous assessments in each clinical subject
  • OSCE scores from internal clinical exams conducted during training
  • Structured feedback records from clinical supervisors
  • Attendance and competency logs from rotations
  • ...are better positioned to identify students at risk of NExT failure early enough to intervene, and to demonstrate to NAAC peer teams that their teaching-learning process is outcome-oriented rather than examination-oriented.

    Accreditation Evidence Under NAAC and NMC

    NAAC's assessment of medical institutions uses Criterion 2 (Teaching-Learning and Evaluation) and Criterion 5 (Student Support and Progression) as the primary lenses for examination quality. Institutions that can present:

  • Internal assessment records aligned with NExT competency domains
  • OSCE results from pre-final year internal clinical examinations
  • NExT Step 1 and Step 2 outcomes as outcome metrics under Criterion 5.2 (Student Progression)
  • ...have a substantially stronger evidence portfolio than institutions that rely on annual theory examination results alone.

    NMC's own institutional assessment framework will increasingly reference NExT outcomes as an inspection data point. Colleges with sustained weak NExT performance will face scrutiny on their teaching quality and resource adequacy.

    Comparing NExT to the FMGE Era

    The Foreign Medical Graduate Examination (FMGE) that NExT replaces was a written screening test with a historically low first-attempt pass rate — often below 20% in some years — for Indian graduates returning from abroad. The low pass rate was partly attributed to genuine gaps in clinical training at some foreign institutions, but also to the fact that FMGE tested material in ways that did not align clearly with any curriculum framework, and its evaluation was not standardised across attempts.

    NExT's CBT format, based on NMC's published competency framework for MBBS, gives candidates a clear target. The competency framework specifies what a graduate should know and be able to do at graduation. The examination tests against that specification. The improvement in first-attempt pass rates that most clinical educators expect from NExT, relative to FMGE, is driven primarily by this alignment — not by the examination being easier, but by it being coherent with what medical schools are supposed to be teaching.

    A National Standard, Digitally Enforced

    NExT is not a digital evaluation story in the narrow sense of on-screen marking for answer scripts. It is a digital evaluation story in the broader sense: a national examination with standardised assessment design, computer-based scoring, item-level analytics, and structured clinical evaluation, replacing a fragmented system of university-controlled, examiner-dependent tests.

    The evaluation of India's future doctors is being standardised through the same logic that is standardising school board marking and university examination processes across the country. When evaluation is high-stakes, automated, and auditable — when the process is as reliable as the credential it produces — the credential itself becomes more trustworthy.

    For the hospitals that will eventually employ NExT-cleared graduates, and for the patients those graduates will treat, that trustworthiness is not an abstract administrative benefit. It is the point.
